When start the HL7 Notepad, as you would expect, you are looking at an empty file. You can use the 'File' menu to open an HL7 message file or click the 'open' button on the toolbar.

If you want to configure your system so that you can double-click a file in Windows Explorer and have it automatically open in the HL7 Notepad, follow these simple steps.
In Windows Explorer right click on an HL7 message file and see the following menu. Choose Open With and then select 'Choose Program' and browse to the installation folder and choose HL7Notepad.exe.
Or conversely you can choose 'Properties' from the menu above to open the MS file properties window (see below). *NOTE that you can choose 'Properties' from within the HL7 Notepad when you have a file opened to achieve the same result.
